Permission Slip for Outdoor Activities

During Seasonal Allergy Time

Dear Mom & Dad,

May your child go outside to play? Our students are offered a period of daily outdoor recreation, weather permitting. Now that fall/spring allergy (pollen, mold, etc.) season has begun, some of the children are advising teachers that they should not be going outside due to allergies. We feel this decision should be made and reported to the school by you, the parents.

Please print then complete the bottom of the page identifying the duration and conditions of your child's outdoor restriction. Then submit form to classroom teacher. Thank you.
